You can take this form to your local imaging or pathology clinic. once you have had the test(s) completed, you will then need to notify Doctors on Demand via phone or email, letting them know where you have had these tests completed so that they can follow up to obtain your results. Once Doctors on Demand have received your results, they will advise the referring doctor.
